Overview

The MDS Coordinator is an RN or LPN who is responsible for overseeing the complex MDS (Minimum Data Set) process. This person must be a diligent leader in resident care planning, regulatory compliance, and Medicare/Medicaid reimbursement. It is imperative that this individual have a high attention to detail, strong communication and collaboration skills, and clinical documentation expertise.

· Schedule all assessments and set appropriate ARD dates

· Complete MDS process, including MDS forms, CAAs, Triggers, and formulate individualized plan of       care for each resident

· Maintain a master schedule of all assessments for the interdisciplinary care team

· Ensure all completions and signatures for MDS and care plan

· Complete additional reviews of validation reports, quality indicators, labs, charts, and more

· Provide hands-on assistance to community residents as needed

· Ensure accuracy of ICD coding

· Monitor Case Mix index

· Participate in committees to improve quality of care

· Maintain compliance with federal and state regulations

· Prepare for audits and inspections

· Other tasks as requested by supervisor

Qualifications

· Valid Registered Nurse or Licensed Practical Nurse license

· Current CPR and First Aid certification or ability to acquire within 60 days

· Knowledge of MDS 3.0 guidelines, ICD coding, and reimbursement systems (PDPM)

· Familiarity with Medicare/Medicaid regulations

· Organization, time management, and detailed documentation skills

· Experience in senior care is preferred

· Resident Assessment Coordinator Certification (RAC-CT) is preferred
