Our Lady of Hope Health Center offers comprehensive, on-site, respiratory therapy to residents. By offering this program, individuals with high intensity cardiopulmonary issues can be treated and monitored while residing on site in their assisted living, nursing, or memory care apartment.
Unlike the traditional model of respiratory care where a variety of different practitioners from an outside provider visit and treat residents, Our Lady of Hope has a dedicated respiratory therapist on staff.
The respiratory therapist is in the building each day, creating patient-specific treatment plans in coordination with the existing care team. This includes frequent communication with the resident’s physician and on-site nurses, resulting in a more coordinated, consistent approach to care. The respiratory therapist also works with any physical, occupational, and speech therapist who is providing care for on-going risk assessment.
Cardiopulmonary education is provided to the rehab team, as well as the resident and their family. This education extends to disease management, equipment use and care, and medication management.
Through this unique cardiopulmonary care and wellness program, we help residents meet their care plan goals and lower the risk of future hospitalizations.
Care can be provided for pulmonary needs due to:
